The 75-year-old was last seen on Saturday, August 4 at an address in Beaufort.
Police and family members hold concerns for John’s welfare as he has a medical condition and have released an image in the hope someone can provide information on his whereabouts.
John is described as 170cm tall with a medium build, balding white hair and a fair complexion.
Police believe he may have visited a care facility in Ballarat on 9 August.
Anyone who sees John or has information on his whereabouts is urged to contact Ballarat Police Station on 5336 6000.
